Good Faith Estimate Disclaimer:

 At CHOOSING Her Wellness Center, we believe in transparency and helping you make informed decisions about your care. In accordance with the No Surprises Act, you have the right to receive a Good Faith Estimate of the cost of services if you are uninsured or not using insurance.   Please note that this is only an estimate; your actual costs may vary based on the frequency and duration of sessions. We do not accept insurance at this time but can provide a superbill upon request for potential out-of-network reimbursement.  For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call (800) 985-3059.

Insurance

INSURANCE

For clients using insurance, we partner with Headway to manage insurance verification, billing, and claims processing. If you choose to use insurance, you will receive separate onboarding steps from Headway to activate your coverage.

It is your responsibility to confirm your coverage, copayments, deductibles, and out-of-network benefits directly with your insurance provider. You are ultimately responsible for all fees incurred for services rendered. If your insurance fails to cover services for any reason (e.g., non-covered services, out-of-network provider, deductible not met), you will be responsible to pay for services. Insurance companies do not cover missed appointments or late cancellations. You will be charged a fee for appointments that are missed or not cancelled 24 hours prior. Review our POLICIES.

Self PAY and Sliding scale

The fees listed below are for self-pay clients only and reflect the cost of services without the use of insurance. These rates are subject to change and may vary depending on the type and length of service provided. Self-pay fees may be eligible for out-of-network reimbursement if your insurance plan offers those benefits. We can provide a superbill upon request to support reimbursement. Therapy sessions are typically scheduled on a weekly basis for 53 minutes.

Sliding scale therapy

We offer therapy sessions at a reduced rate for individuals through the Open Path Psychotherapy Collective. To receive sliding scale rates through Open Path, clients must register as an Open Path member (https://openpathcollective.org). Open Path charges a one-time lifetime membership fee; once registered, you can access our reduced-rate sessions as long as you remain an active Open Path member. Spots are limited.

QUESTIONS TO ASK YOUR INSURANCE

 Please contact your health insurance provider to verify how your plan compensates you for psychotherapy services. Below are some questions you could ask your insurance provider to help determine your benefits:


  • Do I have a deductible? If so, what is it and have I met it yet?
  • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
  • Do I need written approval from my provider in order for services to be covered?

 Please contact your health insurance provider to verify how your plan compensates you for psychotherapy services. Below are some questions you could ask your insurance provider to help determine your benefits: 


  • Do you cover out-of-network therapy services? 
  •  What is the reimbursement rate for out-of-network therapy?
  • What is the reimbursement schedule? 
  •  What is the process for submitting a claim for out-of-network therapy? What documentation do I need for reimbursement?
  •  Are there any limits to the number of sessions covered for out-of-network therapy?
